Back pain that eases when you move is not behaving like most back pain, and that single difference is often the first clue to ankylosing spondylitis (AS). A disc or a strained muscle usually complains more with activity and settles with rest. AS pain does the reverse: it gathers during rest, peaks on waking, and loosens once you start working and walking.
AS is often first mistaken for a muscle strain, a disc problem or poor posture. This post gives you five signs we look for in the clinic, described in the words patients actually use. No single sign is a diagnosis. Together they tell you when to stop treating the pain as mechanical and get it investigated.
1. Someone in your family has it, or something close to it
When we take the history, we ask first whether a parent or sibling has AS, and whether anyone in the family has another autoimmune or joint condition. We ask it carefully, not as a formality. Family history does not make the diagnosis, and its absence does not rule AS out, but it changes how seriously we read the other four signs.

2. Your back has stopped doing ordinary things
AS shows itself as lost movement, not only as pain. When the lower back is involved, three everyday movements give it away:
- turning from side to side while lying in bed
- getting up from a low seat
- bending forward to tie your shoelaces
Notice whether you have quietly changed how you do any of these. AS can also reach other parts of the spine: in the neck, the early difficulty is turning the head while driving, and in the mid (dorsal) spine it is a reduced chest expansion on a deep breath. Because this post is about back pain, we stay with the first three.
3. The pain sits like a belt and does not travel
Patients describe this sign in one phrase: pain in the belt region, as if something is tied around the area. It sits near the sacroiliac (SI) joint, where the lower spine meets the pelvis, and it stays there without radiating down the leg.
That last detail matters. Pain that travels along the leg usually points to a different problem, a nerve being pressed, and we come back to it below.
4. It is worst when you wake up, and better once you move
This is the sign that separates AS from mechanical back pain most clearly. The pain and stiffness are worst on waking. Patients say it plainly: it takes twenty to thirty minutes to come out of bed, and as soon as they start working and walking, the pain gets better. Rest makes it worse, not better.
Mechanical pain, from a disc or a strained muscle, usually runs the other way.

If your back is stiffest after you have been still the longest and loosens with use, do not file it under posture.
5. It is not only the back: the hip and the knee join in
The joints that most often join the back are the hip and the knee. A hip or knee that aches or stiffens alongside belt-like back pain should not be filed as separate arthritis. Read together with the other four signs, it strengthens the picture.
Leg pain in AS is a consequence, not the disease itself
AS does not cause sciatic pain directly. When leg pain appears in a person with AS, it comes secondarily: the restricted movement in the back can end up compressing the nerve. That makes it a mechanical problem, separate from the AS inflammation, and it needs its own assessment. This is why the belt-like pain without radiation in sign 3 remains the AS pattern. One warning applies whatever the cause. If leg pain comes with loss of control over the bladder or bowel, or numbness around the groin or inner thighs, treat it as an emergency and go to a hospital the same day. Read when sciatica is a surgical emergency before you wait on any other treatment.
What Ayurveda calls it, and why the stiffness builds
In Ayurvedic diagnosis, AS is an Aamaja Gambheera Vatarakta, and each word does a job:
- Aamaja names the overall stage: the disease is active.
- Gambheera names the location: deep in the joints.
- Vataja names the dosha surge that drives it, affecting the Majja dhatu.
- Vatarakta names the condition itself.
You may meet it written as "Vataja Vatarakta" elsewhere. It is the same condition in different wording.
Like many other autoimmune conditions, AS also brings changes in the small blood capillaries. Together, these build the stiffness you feel when you wake. A normal X-ray does not close the question
We usually start with an X-ray. When it shows changes, we go on to HLA-B27, CRP, RA factor, ESR and related tests. If the X-ray is normal but the symptoms are strong and there are valid reasons to suspect AS, we investigate with these tests anyway. A clear film is not the same as a clear diagnosis.
No single test decides it, and a positive HLA-B27 on its own is a clue, not a verdict. The results are read together with your history and the five signs above.
