Classical Panchakarma · Snehana procedure

Pizhichil

पिझिचिल् — "squeezing". Warm medicated oil, wrung continuously over the whole body.

Four therapists work in coordinated rhythm for 45–60 minutes without a break, dipping cloth in warm taila, wringing it over the body, working the oil into tissue by hand. It is not a massage, and it is not a spa treatment with a Sanskrit name. It is an oleation procedure with defined parameters, defined indications, and a defined condition under which it must not be performed.

We do not sell packages. We treat diagnoses.

Pizhichil has been sold badly for a long time. It appears on menus as a luxury item, priced by the hour, offered to anyone who walks in. That version of the procedure is not the procedure. What follows is how it is actually performed when it is being used to treat a diagnosis.

Pizhichil at Sukhayu Ayurved is never a standalone admission. Before anyone is placed on that table, there is a diagnosis, a prakruti assessment, and a decision about where this procedure fits in the sequence of that patient's treatment.

Why Pizhichil, and not something local

Most Panchakarma procedures are local. Oil applications, localised swedana, targeted therapies — they are precise and they are appropriate.

But a great many chronic conditions are not local. Vata, when it is aggravated, does not stay where you found it. It moves. A patient presents with a lumbar complaint and, on examination, the dryness is systemic, the stiffness is not confined to the back, the sleep is broken, the nervous system is running hot. Treating the lumbar spine alone in that patient is treating the loudest symptom, not the disease.

Pizhichil is the answer to that problem. Continuous warm oil over the whole body, sustained for the better part of an hour, delivers oleation and heat systemically rather than at a point. This is why the indication list reads the way it does — it is a list of conditions that share a single characteristic: they are not confined.

Clinical scope

Indications

That list spans neurology, rheumatology, orthopaedics and psychiatry — which looks like overreach until you notice what connects it. Vata governs movement, nerve conduction, dryness and degeneration of tissue, and the restlessness of an unsettled mind. Aggravated Vata is the common thread; these indications are the map of where it does its damage.

Neurological

Neurological disorders Hemiplegia Sciatica

Rheumatic & autoimmune

Rheumatic diseases Joint pain & muscle stiffness

Spine, bone & muscle

Musculoskeletal disorders Diseases of bone & joints Osteoarthritis Intervertebral disc prolapse Painful conditions of head & neck

Mind, sleep & systemic

Insomnia, anxiety, stress Respiratory disorders Generalised (systemic) disease Vata aggravation Dosha imbalance Skin dryness

The procedure is applied differently depending on which of these you are treating. Pizhichil in hemiplegia is not Pizhichil in insomnia. The oil differs, the positioning differs, the surrounding protocol differs. The procedure is a method, not a fixed recipe.

Definition

The six parameters that define a real Pizhichil

Change any one of these and you are performing a different procedure. This is the line between a clinical Pizhichil and an hour of warm oil.

01 The oil
There is no single Pizhichil oil. The medicated taila is selected according to the patient's prakruti and diagnosis — both, not one or the other. Two patients with the same diagnosis and different prakruti receive different oils. Two patients of similar prakruti with different diagnoses receive different oils. A centre that uses the same oil on everyone who books is dispensing, not selecting.
02 Temperature
Warm — and warm has a range. Too cool and the oil sits on the skin instead of penetrating. Too hot and it is uncomfortable and counterproductive, and in some conditions unsafe. The temperature is maintained continuously across the full session, which is one reason the procedure requires four sets of hands and constant attention.
03 Four therapists
Not two. Not one working around the table. Four, in coordinated rhythm, so the oil arrives continuously and evenly and the warmth never drops on one part of the body while another is being worked. Fewer than four is not the procedure.
04 Positions
The patient is moved through defined positions across the session so that the whole body receives the oil and the therapists can work each region. Positioning changes with the indication — the sequence used for a disc prolapse is not the sequence used for hemiplegia.
05 Timing — why evening
Evening is chosen by protocol, not preference. There should be a stated reason for the timing of any procedure; if a centre cannot give you one, the timing is being set by the appointment book rather than by the treatment.
06 Duration of course
45–60 minutes per session, and a course length determined by the patient's condition — not by a package. Who decides the number of days is the diagnostic question: a physician, based on your diagnosis, or a price list?

The session

Inside the hour, and the hours after

Before

Assessment and preparation

Ama is ruled out or cleared first. Where Kapha is involved, Choorna Pinda Swedana may be alternated with Pizhichil rather than oil being applied over an unprepared body. The oil for that day is selected against your prakruti and diagnosis.

During · 45–60 minutes

Continuous oleation

Cloth is dipped in the warm taila, wrung out over the body, and the oil is worked into tissue by hand in the same motion — without a break, with four therapists in rhythm and temperature held steady throughout.

After · the same day

The instructions are part of the treatment

Avoid exposure to cold: no cold water, no cold wind, no cold food. The body has just spent an hour being warmed and oiled with intent, and cold exposure reverses it. A patient who receives an excellent Pizhichil and then eats a cold meal in an air-conditioned room has received less than half of it.

Across the course

Measured over days, not sessions

Improvement in chronic conditions is measured over a course, not a session. Documented outcomes at Sukhayu are individual cases; what your protocol achieves depends on your diagnosis, the stage at which you present, and your adherence.

Safety

When Pizhichil must not be done

Absolute — the procedure is withheld

  • Where ama is present and has not been ruled out or cleared. Oleation over ama drives it deeper; this is the single condition under which the procedure must not be performed.

Conditional — the protocol is altered

  • Where Kapha is involved, Pizhichil may be alternated with Choorna Pinda Swedana rather than given alone.
  • Where the indication calls for a different route or a local procedure, Pizhichil is not selected simply because it is available.
  • Any decision here follows examination — not enquiry, and not a booking.

Placement

Where Pizhichil sits in your treatment

Always inside a larger, diagnosis-led protocol. It is not something you book. It is something that is prescribed, if the diagnosis calls for it, at the point in your protocol where it belongs.

Step 01

Consultation

Step 02

Examination & prakruti assessment

Step 03

Diagnosis

Step 04

Pizhichil, if indicated

Step 05

Course review & after-care

Which means that before Pizhichil is ever discussed with you, there has been a consultation, an examination, and a diagnosis. That sequence is the treatment. The hour on the table is where it becomes visible.

Honestly

What to expect

Patients ask what it feels like. It is deeply relaxing — the warmth, the rhythm, the four sets of hands working in coordination. Many patients sleep through part of it. That is a real and useful effect, particularly in the insomnia and anxiety indications, where the relaxation is not a side benefit but a therapeutic aim.

But relaxation is not why it is prescribed for a disc prolapse or for hemiplegia. In those cases you are receiving a systemic oleation and thermal therapy that is doing structural work on tissue, and the pleasantness of the hour is incidental to the reason it is being done.

Cure is divine. Treatment is possible.

Pizhichil is one procedure within a treatment that is possible. Ayurvedic care at Sukhayu Ayurved is complementary to your conventional treatment, not a replacement for it. Any decision about changing or tapering medication rests with your prescribing physician and no one else.

Due diligence

Questions worth asking any centre offering Pizhichil

Before you book Pizhichil anywhere — here or elsewhere — ask these seven.

  1. Who assessed me for ama?

If the answer is nobody, walk away.

  2. How was my oil selected?

If the answer is that they use one oil for Pizhichil, it is not being selected.

  3. How many therapists?

Fewer than four is not the procedure.

  4. What is the temperature protocol?

There should be a specific answer.

  5. What time of day, and why?

    There should be a reason.

  6. How long is my course, and who decided?

A physician, based on your diagnosis — or a price list?

  7. What is this procedure treating?

If the answer is "wellness," there is no diagnosis, and without a diagnosis there is no treatment.

Frequently asked

Pizhichil: common questions

Is Pizhichil the same as an Ayurvedic massage?

No. Abhyanga is a massage. Pizhichil is an oleation procedure in which warm medicated oil is squeezed continuously from cloth over the whole body while four therapists work in coordinated rhythm. The oil, the temperature, the positioning and the duration are all set by the diagnosis.

Can I book Pizhichil directly as a single session?

Not at Sukhayu Ayurved. It is prescribed after consultation, examination and diagnosis, and only when the diagnosis calls for it. A single session sold on its own is a spa item, not a treatment.

How many sessions will I need?

Course length is determined by your condition and decided by the treating physician after examination — not by a fixed package. It is discussed with you once a diagnosis has been made.

Is there anyone who should not have Pizhichil?

Yes. It must not be performed where ama is present and has not been cleared, and the protocol is altered where Kapha is involved. Suitability is established by examination.

What should I avoid on the day of the procedure?

Cold exposure of any kind — cold water, cold wind, cold food. These instructions are part of the treatment, not an optional extra.

Consultation

A procedure is chosen after a diagnosis, not before one

If you are living with a chronic Vata-predominant condition — a rheumatic disease, a neurological disorder, a disc prolapse, motor-neuron disease, osteoarthritis, or long-standing musculoskeletal pain — a consultation will establish what your protocol should contain.
